Are prostheses covered by insurance?

Most insurances companies require medical necessity to consider coverage for prosthetic care. Therefore, please provide our office with a letter of medical necessity from your Ophthalmologist (or other In-network provider) along with a copy of your current insurance identification card and we will be happy to verify your eligibility, benefit coverage, and authorization requirements.

We accept check, cash, or credit card for coinsurance, deductible, and copayments. We utilize Apple Pay and Google Pay as well.
